Travel Advice and Immunisation

Please use our Travel Risk Assessment form to book your travel vaccinations.

For those patients wishing to access travel vaccinations please bear in mind for many vaccinations to be effective they need to be given eight weeks prior to travel.  Therefore please give sufficient time by requesting a travel appointment at least 8 weeks before your planned travel date. Generally, we have appointments available 8 weeks in advance but if you are booking late or if we do not have any nurses appointments available you will need to attend a private travel clinic.

Travel to exotic locations, in some cases multiply locations is the norm these days, but careful consideration needs to be given by the nurse to what vaccinations are required, therefore travel appointments are 30 minutes in duration and we can see two family members at a time. If more than 2 are travelling then 2 appointments will need to be made.

When booking the appointment you will be asked to complete a travel questionnaire, in order for the nurse to determine which vaccinations you need prior to your appointment. It is also important to remember that only Diphtheria, polio and tetanus, typhoid and Hepatitis A vaccinations are available free on the NHS.

If you are unable to attend an appointment, please contact us to cancel the appointment so that we can treat another patient/s.

NHS Health Checks and Long Term Condition Reviews

What is an NHS Health Check?

It is a regular check-up for people aged between 40 and 74 who aren’t already receiving treatment for a long term condition.

The aim is to spot any early signs that could increase your risk of stroke, kidney disease, type 2 diabetes or dementia.

As we get older, the chances of developing any of these conditions go up. Regular checks can identify risk factors and help you make simple changes to your lifestyle to help reduce those risks.

What happens during your appointment?

An NHS Health check consists of two appointments. The first one is a blood test and then roughly a week after having the blood test you will have a 30 minute appointment with a Nurse who will ask some simple questions about your lifestyle, including your diet, how much you drink or smoke and your exercise habits. We will also record your height, weight and blood pressure.

After the check, you can discuss any changes you could consider with the Nurse to help reduce your risk factors. Often, simple changes can make a big difference.

How do I arrange one?

Typically, you’ll be invited for an NHS Health Checks once you turn 40, and then every five years after that. We will send you a letter inviting you to make an appointment when it is your turn.

You can find out more about NHS Health Checks at healthcheck.nhs.uk

What is a LTC review?

The following are classed as long term conditions:

  • Asthma
  • Diabetes
  • CHD (Chronic Heart Disease)
  • COPD (Chronic Obstructive Pulmonary Disease)
  • CKD (Chronic Kidney Disease)
  • CVD (Cardio Vascular Disease)
  • Dementia
  • Heart Failure
  • Hypertension
  • Learning Disabilities
  • Mental Health
  • Pre-Diabetes
  • Rheumatoid arthritis
  • Stroke & TIA

It is essential that we regularly see you to review your condition.

When you come to the practice, you can talk with a clinical team member about how you are getting on and any medication you are taking.

The aim is to make sure your condition is being controlled as well as possible and to identify any areas where we can make changes to help you. This might include changes to the type or dosage of medication and check any equipment you might use, like inhalers, are working effectively.

We aim to review most long term conditions every 12months. When it is time for your review, the practice team will be in touch about making an appointment.

We will be calling for you during your month of birth for your review – therefore you may be called in earlier than your previous review date and in some circumstances may be called for more than once during this coming year.

Multiple Long Term Conditions

For patients who have more than one long term condition, we take a slightly different approach. Rather than inviting you to make separate appointments for each condition, we will make one longer, combined appointment.

We can take a comprehensive look at your overall health rather than focusing on each condition in isolation. By looking at everything collectively, we can ensure you get seamless access to the integrated care you need.

We will normally send you an invitation in the month of your birthday.

Health Visitor

It is with regret that we must inform you that Childrens Clinics which were provided here by our Health Visiting Team have been transferred to Somercotes Childrens Centre which is sited next to Somercotes Nursery and Infant School. Their sessions are 13:00 to 14:30 every Tuesday.

The Health Visitor Teams have been re-organised by their managers which also means that some families will have different health visitors.

Our current Health Visitors will retain families living within Somercotes only.

It should be noted that most of the Health Visitors share an office based at Alfreton Health Centre and our patients first point of contact remains on 01773 546800.
